• All information provided on this form must be true and accurate to the best of my knowledge.
• I understand that submitting this form does not guarantee assistance. Distribution is based on the availability of donated items and the number of families our program can serve.
• Applicants are required to upload proof of the child’s identity (such as a birth certificate or Medicaid card).
• Applicants must also upload proof of residency (utility bill, lease agreement, or other official document).
• Incomplete applications or missing documentation may result in a delay or denial of assistance.
• Once submitted, you will receive a response within 2–3 business days. We kindly ask for your patience during this process.
